Enter your Employee Insurance No.
STEP 4 - ESIC 7B Form Download Step 4
After submitting the Employee Insurance No you will see all th information of that Employee on the screen. There you will find an option in Blue Colour to download the Medical Acceptance Card. Just click on that option and the ESIC 7B Form or the Medical Acceptance Card for that particular employee will get downloaded.
